To help us maintain accurate and up-to-date medical records for every student, all families, including returning students, are required to submit updated medical forms each school year. Complete and accurate records allow us to provide the best care in both everyday and emergency situations and help support a safe and healthy environment for our entire school community.
All required medical forms will be distributed and submitted through Brightwheel, our student management system. Brightwheel allows families to access, complete, and upload forms directly from a phone or device. All forms must be submitted via Brightwheel by August 1. Forms are in Brightwheel under Paperwork Forms.
All required forms are also linked below and may be downloaded, completed, and emailed to the school office at kpoe@redeemerbaltimore.org should there be any difficulties with Brightwheel.
If your child has a physician-documented allergy, asthma, seizure disorder, or other medical condition requiring medication or medical action plans at school, additional forms are required for your child to begin school. Please review the Additional Medical Forms & Medication Requirements document for important information regarding required forms, medication submission procedures, and medication requirements. All forms and medication are due to the school office by August 1, 2026.
