New for 2025–2026: All students, including returning families, must submit a completed Health Inventory Form to ensure we have the most current medical information and can provide proper care.
Please Note: All required fields are highlighted. Incomplete forms will be returned. To avoid delays, ensure all highlighted sections are filled out and required signatures are provided.
*If your child has a physician documented allergy or asthma condition, additional forms are required in order for your child to start school. Please click here for specific instructions and information related to completing the forms. All forms are due August 1.
Health and Emergency Forms are available through Brightwheel and can be completed and submitted directly in the app. If preferred, forms may also be downloaded, printed, and emailed to kpoe@redeemerbaltimore.org.
Please note that students may not begin school unless all required medical forms are submitted, including any additional forms for allergies or asthma.
